Daybreak - Adult Day Center 

An Adult Day Center provides a safe, engaging environment for seniors who cannot be left alone for extended periods. These programs help individuals stay active, improve health, and reduce isolation while giving caregivers a much-needed break.

Daybreak - Eligibility and Cost 

• There must be a caregiver living in the home. 
• The person being cared for must be 60 or older.  
• Costs are based on a sliding scale according to income. Proof of income is required to receive services on a sliding scale. (for sliding scale must reside in Wexford County) 
• Financial assistance is available through grants and long-term care insurance. 
• If you qualify for care management or Medicaid waiver services through the Area Agency on Aging, or if you are eligible for programs through DHHS, we collaborate with these organizations to provide care in Wexford County. Please consult your caseworker to determine if we can work together. 
• Services are provided Monday - Friday, 8 AM - 4 PM, depending on availability.
